Insurance
This office is contracted as a participating office with the plans listed to the right. It is the patient's responsibility to be familiar with his or her particular insurance plan and to bring the most current insurance card to each visit.

Payment
A copy of our Financial Policies will be given to each patient entering the practice. The most current copy of your insurance card needs to be brought to each visit. It is the responsibility of the patient to pay any part of the bill not covered by insurance at the time of the visit. All co-pays, deductible amounts, co-insurance, or non-covered services are due at the time of service.
