form of indentification with a picture ID to each visit. |

If you are unable to keep an appointment, please call to

Refills
Ask for any medication refills will be needed before your
next visit. |

Insurance
Copayment
If your insurance plan has a deductible, co-insurance, or co-pay, that
amount will be due at the time of the visit. |

talk to the doctor about (it may not be possible to address all of your
